Getting Started is Simple!
Step 1
Fill out your Health Questionnaire packet.
The Health Questionnaire packet is extremely important in the diagnosis and treatment plan that Dr. David J. Bilstrom and Bradly K.L. Brown, PA-C, will prepare for you.
Please mail or fax the completed Health Questionnaire packet to us once you have it filled out. If we DO NOT have your completed questionnaire in our office five days prior to your appointment, we will call to reschedule your appointment.

Practice Policies for Patients
Information about Bingham Memorial’s International Autoimmune Institute and Center for Functional Medicine is available through our website at {new landing page link here}.
Medical Records can only be released with your authorization. You are responsible for obtaining previous medical records from other physicians or health care providers who are not affiliated with Bingham Memorial. Please contact your physician or other health care providers to obtain these records. Your records should be express mailed to the following address:

Initial Visits
Many of the tests require a 10-hour fast. You can and should drink water during this fast and take all prescription medication. Costs of all testing will be reviewed with you by our staff after your medical consultation before labs are drawn. You will receive all final lab results and be guided through their interpretation at your follow-up visits.
Confirmation and Cancellation of Appointments
If you must cancel or re-schedule your appointment, please contact us at least three days prior to your appointment at (208) 782-2444.

Arriving for Your Appointment
Please make sure to arrive 15 to 30 minutes early to your appointment to make sure we have all needed information. Make sure to bring your insurance cards and a driver’s license with you so we can scan them.

What to Expect at Your Appointment
Office Check-In
- Please arrive 15 to 30 minutes prior to your appointment time.
- Update personal information and sign consent forms.
- Meet nurse to have vital signs taken.
1st Appointment
60-minute appointment
Detailed review of history
Physical examination performed
Lab work & food tests ordered
Begin development of initial treatment plan
2nd Appointment – 6 Weeks Later
60-minute appointment
Review improvements made so far
Review together all lab results
Complete initial treatment plan
3rd Appointment – 4 Months after 2nd Appt.
60-minute appointment
Review improvements made so far
Review together all new and follow-up lab results
Modify treatment plan as needed
Final lab results

Step 5
Insurance Information
As part of your appointment, you may see our health and wellness counselor. As a courtesy, we will contact your insurance for prior authorization before the visit you have scheduled with our counselor. Please provide your insurance information and a copy of your card within 10 days of receiving this email.

Prescription Refills
It may take up to 24 business hours to process a prescription refill. Please plan ahead to avoid any interruptions in your medications. Prescription refills can be faxed to our office at (208) 782-2444 by your pharmacy.
